Steps to Get Medically Cleared

Here are the basic steps to get cleared to play. Please see the section below for more detailed instructions.

  1. Update your SportsWare Online account
  2. Download and complete your Medical Packet
  3. Physical exam with your primary care physician
  4. Baseline concussion test
  • The Head Athletic Trainer must certify the steps above before you obtain medical clearance. You still need to ensure compliance clearance before you are fully cleared to play.

 


  1. Updating SportsWare Online
    1. Login to your SportsWare account
    2. Update ALL OF THE FOLLOWING sections;
      1. MY INFO
      2. MED HISTORY
      3. COVID-19
      4. PHQ-9
  2. Completing your Medical Packet
    1. Download and print out the medical packet
    2. You will need to COMPLETELY fill out pages 3, 4, and 6 through 9 of the medical packet.
    3. You will also need to complete the top of the Athletic Physical Form (page 5) that you will provide to your doctor for the physical exam.
    4. If you have been vaccinated against COVID-19, please provide a copy of the front and back of your CDC vaccination record card as proof
  3. Get a physical exam with your primary care physician
    1. Your primary care physician needs to FULLY complete both the Orthopedic Screening and General Medical Exam sections of the Athletic Physical Form (page 5)
    2. Ensure they sign and date the form along with an office stamp in the lower right-hand corner
  • Submitting your medical packet: Completed medical packets can be scanned and emailed to  amorrison2@sjny.edu or faxed to (718) 940-5305
    • IMPORTANT: You will still need to submit hard copies of the ORIGINAL medical packet to the Sports Medicine staff in the Athletic Training Facility in The Hill Center
  1. Baseline Concussion Test
    1. Once you have updated your SportsWare Online profile and submitted your FULLY completed your medical packet, you will be sent an email with instructions on how to take your baseline concussion test
      • The test is conducted online and can be completed from home
